Can you live normally without a gallbladder?

Most people do. The published figures put a number on the minority who have lasting trouble, and on how long it usually takes to settle.

Most people live normally after their gallbladder is removed, because the liver continues to produce bile and it drains into the intestine continuously instead of being stored. The gallbladder is a reservoir, not a factory. It stores bile made by the liver and concentrates it, then squeezes it into the small intestine when a meal arrives. Remove it and the liver carries on producing bile exactly as before; what changes is the delivery schedule, from pulses after eating to a continuous trickle.

For most people that change passes without consequence. For a minority it does not, and the published figures are specific enough to be useful.

Postcholecystectomy syndrome is the term for abdominal pain, indigestion or biliary symptoms that persist or appear after the operation. The meta-analysis that produced the 3.21 percent figure also found that patients who had complications or symptoms before surgery were substantially more likely to have symptoms after it — which is a reason for careful assessment before operating rather than evidence about the operation itself.

What the long-term national data show

Korea's national insurance database has produced a series of studies reporting higher rates of later diagnoses in people who have had a cholecystectomy, including cancer, type 2 diabetes and chronic kidney disease. Those findings are real as measurements and weaker than they look as evidence of harm.

The largest of them, comparing 715,872 operated patients against 1,431,728 matched controls, found the raised rate of hepato-biliary-pancreatic cancer diagnosis was highest one to three years after surgery and had returned to the control level after five to ten years. A cause that worked through altered bile physiology would be expected to build over time rather than fade, and the two groups differ both in why they needed surgery and in how closely they were subsequently watched.

What is not established

  1. The diarrhoea review found no agreement across its 21 studies on who is likely to develop it; sex, age and weight were each proposed by some studies and not confirmed by others.
  2. The syndrome meta-analysis drew heavily on Chinese-language databases and pooled nine studies, so its 3.21 percent should be read as an estimate with real heterogeneity behind it rather than a settled rate.
