Will thyroid surgery change my voice?

The figure patients are usually given describes nerve injury. A Korean study of 527 people whose nerves were verified intact found their voices changed anyway.

Voice change after thyroid surgery is common and usually temporary, and it happens even when the nerves are undamaged: in 527 patients with intact vocal fold movement and normal nerve conduction, pitch fell immediately after surgery and self-rated voice handicap rose before returning to near-baseline by six to twelve months, while permanent nerve injury itself occurs in roughly 1 to 2 percent of operations.

The answer most patients are given is a number for nerve injury — commonly one or two percent. That number is roughly right and it answers a narrower question than the one being asked. A voice can change without a nerve being damaged, and the evidence on that is more recent and less often quoted.

What nerve injury actually runs at

The recurrent laryngeal nerve runs immediately behind the thyroid and controls the vocal cord. A meta-analysis pooled 199 studies covering 304,352 patients to establish how often it is injured, and found the answer depends heavily on which operation is being done.

By surgical approach, transoral and transcervical routes carried the highest transient injury rate at 5 percent and the transaxillary route the lowest at 1 percent. Permanent injury was highest after secondary thyroidectomy — an operation on a neck that has been operated on before — at 2 percent.

The number depends on who gets examined

A Korean series of 1,000 consecutive single-port transaxillary robotic thyroidectomies reported transient nerve injury in 4.1 percent and permanent injury in 0.2 percent. The pooled estimate for the transaxillary approach is 1 percent.

The Korean series confirmed nerve injury by laryngoscopy in every patient. A series that scopes only the patients who complain of a hoarse voice will find fewer injuries, because some vocal cord palsies do not produce a voice the patient notices. The higher number is not a worse result. It is a more complete count, and this publication has reported that comparison in detail.

Voices change without nerve injury

A Korean tertiary centre followed 527 patients who had normal vocal fold movement on laryngoscopy and normal electromyography two to three months after thyroidectomy — that is, patients whose nerves were verified to be working. Voice was measured before surgery and at three days, two weeks, and one, three, six and twelve months afterwards.

What this does not tell you

  1. The 527-patient study is one tertiary centre and retrospective. It establishes that non-neural voice change occurs and is measurable; it does not give a Korean population rate.
  2. Acoustic measurements are not the same as a person disliking their own voice. The study captured both, and the self-rated scores recovered further than some of the acoustic ones did.
  3. None of these studies randomised anything. Patients having completion or revision surgery differ from patients having a first hemithyroidectomy in ways beyond the operation.
  4. Nothing here addresses whether an operation was needed. For low-risk papillary thyroid cancer, Korea now has a guideline covering active surveillance instead, and the voice risk of an operation is one of the things weighed against monitoring.

The defensible summary is that permanent voice damage is uncommon and depends strongly on which operation is done, while temporary voice change is common enough that a patient should expect it rather than be surprised by it — and that being told the nerve is intact does not mean the voice will be unchanged.
