Whether a prostate operation leaves a man incontinent is partly decided before he reaches theatre

Pooled data on 12,620 men finds age, urethral length and existing urinary symptoms predict continence after robotic prostatectomy. All three are measurable at the clinic visit where the decision is made.

In a meta-analysis of 48 studies covering 12,620 men, urinary incontinence in the first three months after robot-assisted radical prostatectomy was independently predicted by age (odds ratio 1.04 per year), membranous urethral length (0.83 per millimetre), International Prostate Symptom Score (1.03 per point), body mass index and prostate volume, with age, urethral length and symptom score still predictive between three and twelve months.

Two articles published here this week reported what Korean prostate cancer data does and does not contain, and both ended at the same gap: neither dataset records continence or sexual function, which are the reasons the decision to operate is difficult in the first place.

That literature exists. It is not Korean, and what it establishes is narrower than a patient wants — but it is more useful than it first appears, because much of what it identifies can be measured before anyone consents to anything.

What predicts incontinence

A meta-analysis pooled 48 studies covering 12,620 men who had robot-assisted radical prostatectomy, and asked which preoperative variables predicted leaking afterwards.

The technique that appears to help

Preserving the nerves that run alongside the prostate protects erections and, to a lesser degree, continence — but preserving them risks leaving cancer behind. NeuroSAFE addresses that trade by examining the removed prostate's surface under the microscope during the operation, so that a surgeon who has spared nerves can go back and remove more if the margin is involved.

A meta-analysis of 13 studies covering 22,183 patients, two of them randomised trials, reports the following against standard robotic prostatectomy.

Better function and fewer positive margins at once is an unusual result, since the two normally trade against each other. The recurrence interval — 0.43 to 1.56 — is wide enough to be uninformative in either direction, which is what a reader should take from it rather than reassurance.

How much weight these numbers carry

Both reviews are unusually direct about their own limitations, and both statements belong with the figures rather than after them.

  • The incontinence review's quality assessment found a high risk of bias in study participation and confounding across the studies it pooled.
  • The NeuroSAFE review reports substantial heterogeneity for some outcomes and notes that most of its non-randomised studies carried a serious risk of bias, which limits causal inference.
  • Eleven of the thirteen NeuroSAFE studies were observational, and surgeons who adopt a technique like this early are not a random sample of surgeons.

What is still missing for a Korean reader

  1. Neither review is Korean, and neither reports how often incontinence or erectile dysfunction occurs — only what makes them more or less likely. A man asking what his own chances are will not find the answer here.
  2. Korea's national datasets record diagnosis, treatment and cost, not function. Until a Korean series reports patient-reported continence and sexual function at fixed intervals, the trade-off at the centre of this decision is undocumented in Korean practice.
  3. The predictors describe robot-assisted prostatectomy. Whether they transfer to open surgery or to radiotherapy is not addressed.
  4. Radiotherapy has its own functional profile, largely bowel and late urinary effects, and none of it appears in these two reviews.
